update mkdfa.pl to pull in pronunciations from dictionnary into .voca file

Update mkdfa.pl to pull in pronunciations from dictionnary into .voca file.

This would reduce transcription errors since pronunciations need to be entered manually in Julian .voca file.

If using a new dictionnary, would not have to update .voca file with new pronunciations.
